New Patient Process
1. SCHEDULE INITIAL CONSULTATION ON WEBSITE
* Patient will be emailed an intake package that must be completed prior to appointment. If not completed appointment will be rescheduled. *
Consultation is conducted via a HIPAA compliant telehealth platform. Initial consultation includes:
– Review of patient’s health history and recent labwork
– Order labwork if needed
– Explanation of services
– Question and answer period
2. POST CONSULTATION
– An invite to the secure patient portal will be sent via email.. The patient portal may be utilized to send HIPAA protected messages, medication information, and forms.
– Invite to the wellness app Body Site will be sent via email for weight loss patients only.
– Invoice for weight loss services or private pay psychiatric services will be sent via email.
– Covered psychiatric services will be billed to your insurance company. You are responsible for any co-pays or unpaid portion of your invoice.
3. WEIGHT LOSS MEDICATION KIT
– Prescription will be processed upon receipt of payment. You will receive an email with a tracking notice as soon as the prescription ships.
– Medication kit will include medication and all necessary supplies.
– Once medication kit is received patient will schedule an appointment with provider for injection instructions and demostration.
